Sarah Hyland, who plays Haley on “Modern Family,” has received a temporary restraining order against her ex-boyfriend, actor Matthew Prokop, claiming he choked her, pushed her and threatened her and her dog.

TMZ published quotes from the document on Tuesday.

“His grip was so tight that I could not breathe or speak,” Hyland, 23, said in a sworn statement. “I was scared and in fear for my life.”

Hyland also said in her affidavit attached to her application for the protective order that Prokop “relentlessly bombarded me with vile, threatening and emotionally disturbing texts and voice mails including his own suicide threats.”

The couple broke up in August, and Hyland says Prokop entered rehab. The actress states she was advised by the rehab director that she should seek a restraining order because of Prokop’s attitude toward her.

According to TMZ, Hyland asked her TV mom Julie Bowen to come to her house to help “peacefully end the relationship,” but when Prokop arrived at the home and saw Bowen and others, he freaked out ... “ran outside into the backyard and began screaming.”

Bowen then told Hyland to get out of the house.

A judge ordered Prokop to stay 100 yards away from Hyland and set a hearing for Oct. 10.

A receptionist for Prokop’s agent Tyler Grasham said they had no comment on the allegations. Prokop is not facing any criminal charges.
